BHDDH’s Mental Health Services include the Community Support Program, which provides case management services and assistance to individuals to meet the goals of their behavioral health treatment plan, as well as access to medical, social, educational and other services; a Mobile Treatment Program, which is an intense case management supportive treatment that provides rehabilitation and support interventions to people with severe and persistent mental illness, enabling them to live in their natural community environments; outpatient programs and services that include individual, group and family counseling, education, as well as diagnostic, clinical and educational services and medication monitoring; residential programs and services, which promote recovery and empowerment; emergency, crisis intervention, and crisis stabilization services, which include behavioral health emergency, crisis intervention, and crisis stabilization services on a short-term basis; and community integration programs and services, which include vocational rehabilitation, psychosocial rehabilitation, supported employment and education, and other community-based rehabilitation services.

Rhode Island Department of Health’s Chronic Care and Disease Management, Asthma Control Program
The Asthma Control Program works toward increasing access to quality health care, asthma education, community resources, and services. One project is “Breathe Easy at Home,” a pilot project designed to diagnose and control asthma triggers in the homes of children with asthma; this web-based referral system enables health professionals to refer their patients with asthma for home inspections conducted by the city’s housing inspectors, in cases where substandard housing conditions are suspected and may be triggering children’s asthma in their homes.

Rhode Island Department of Health’s Chronic Care and Disease Management, Diabetes Prevention and Control Program (DPCD)
DPCP works to target and control diabetes and prevent diabetes-related complications. Programs within the DPCD include the Outpatient Education Program, which trains and certifies nurses, dietitians, and pharmacists to provide diabetes management education to people with diabetes; the Multicultural Diabetes Education Program, which provides multilingual and multicultural diabetes education and support in communities with limited access to formal diabetes education programs; the Diabetes Prevention Program, which provides physicians with tools to improve screening and quality of care for patients at risk for diabetes and to connect patients at risk for diabetes with community resources; TEAMWorks, a 3-hour diabetes self-management program for adults with diabetes; and Living Well Rhode Island, which provides evidence-based chronic disease self-management education to people with diabetes statewide.

Rhode Island Department of Health’s Disability and Health Program
This program strives to promote the health and wellness of Rhode Islanders with disabilities, prevent secondary conditions resulting from a disability and/or chronic health condition, and reduce health disparities between people with and without disabilities. One priority area is the administration of community-based health promotion initiatives and environment modification for people with disabilities; initiatives include “Live Well RI,” a series of eight-week health promotion sessions offered to clients of the Rhode Island Vocational Rehabilitation Program focusing on healthy living and physical activity; a summer health promotion/leadership development program for adolescents and young adults with disabilities; and through a contract with “Access2Adventure,” several physical activity and recreation opportunities for people with disabilities and their care givers.

Rhode Island Department of Human Services’ Children with Special Needs
Services for children with special needs include the RI Medical Assistance Program, paying for medical and health related services for Children with Special Health Care Needs who qualify; CEDARR Family Centers, providing support and respite services to qualified families with children with special needs, and to the children Home Based Therapeutic Services, Kids Connect (specialized services at licensed childcare centers), and Personal Assistance Services and Supports; an Early Intervention Program, promoting the growth and development of infants and toddlers who have a developmental disability or delay in one or more areas; and Katie Beckett eligibility that considers only the child’s income and resources, not the parents’, when determining Medical Assistance eligibility, which enables certain children (under age 19) with long term disabilities to be cared for at home instead of in an institution.

The Office of Rehabilitation Services helps individuals who are deaf or hard of hearing obtain services to meet their vocational goals. Services for the blind and visually impaired include: counseling and guidance, advocacy, vocational guidance, and a summer camp program for children (birth through 21) with visual impairments and their parents; medical and educational planning; for adults over 21, assessment, counseling, and guidance; training in mobility, daily living skills, and communication; vocational training; adaptive equipment; job development and placement; assistance regarding financial and medical benefits, housing, property and income tax exemptions, and information and referral; an independent living program for blind and visually impaired individuals over 55 including specialized services for people who are deaf/blind; and a comprehensive newspaper reading service accessible through the use of a touch-tone phone.
