AAHD and the Lakeshore Foundation have joined 65 organizations support of the Partnership To Align Social Care comments to CMS on the proposed Medicare Physician Fee Schedule. The comments address a wide array of topics including: Community Health Integration and Principal Illness Navigation for Behavioral Health; Caregiver Training Services; Proposed Efficiency Adjustment; and Prevention and Management of Chronic Conditions.
